Male, 83 y-o,

Type 2 diabetes, Ischemic heart disease (PTCA) and dyslipidemia.
Left Critical limb ischemia. Previous amputation of the V toe. Rest pain and TcPO2: 11 mmHg.
Previous antegrade attempt to treat ATA, fails.
Retrograde access followed by ante-retro

Strategy:

Antegrade 6F access.
Distal AT retrograde access (micro-punture set – CXI support catheter), endoluminal-subintimal recanalization of AT.
Hydro ST guide wire. Advance LP Balloon PTA at 2.5-3 mm.
PTA of SFA and POP 5 mm.
