    Type 2 diabetes mellitus.  
    dyslipidemia.
    Ex-smoker.
  • HISTORY:
    Dilated cardiomyopathy with no significant coronary lesions in 2013 .
    LVEF = 42%, permanent AF
    end-stage renal disease.
    chronic ethylism,axono-myelinic-polyneuropathy,tubulo-villous adenoma,hypothyroidism, diabetic retinopathy.
  • Preoperative coronary angiography (AVF)

  • Step-by-Step optimal preparation of the FFR system to avoid drift.
  • Step-by-Step optimal FFR recording.
  • Optimal lesion preparation with rotational atherectomy.

Procedure description

  • Flushing the FFR system with saline and wait for at least 30 to 60 seconds.
  • Putting the system in horizontal position to avoid drift.
  • Connecting the FFR system.
  • Getting epicardial vasodilatation with nitro.
  • Pressure equalization in guiding catheter.
  • Baseline FFR recording.
  • FFR recording with  maximal hyperemia (100µg ) for RCA.
  • Predilatation with NC balloons.
  • Rotational atherectomy.
  • Stent implantation Xience Alpine

Protocol

  • Exposure time: 22min.
  • Radiation dose: 1.66 Gy.
  • Contrast volume: 175ml (Iomeron).
