                     •150ml injected into the left carotid artery
                     •100ml injected into the right carotid artery

•Right carotid endarterectomy with patch
CABG
•Double right carotid restenosis and left de novo carotid artery stenosis

Planned procedure

•Right femoral access
Placing a filter (EMBOSHIELD) above the carotid distal lesion (protecting method)
•Predilatation of tandem lesion
•4-9x21mm ADAPT stent placed distally
•49x31mm ADAPT stent placed proximally
•Post diatation
 

Learning points

•Management of patients with simultaneous or concomitant carotid and coronary artery lesions
Role of carotid endarterectomy before coronary by-pass graft
Importance of regular follow-up after surgery to detect new atheromatous lesions
•Treatment of bilateral carotid artery lesions by carotid stenting
