Right internal carotid stenosis Left internal carotid stenosis (tandem lesion)

Mr W. - 76 years old

Bilateral Asymptomatic carotid artery stenosis

Reasons for carotid revascularization:

  • Bilaterality of lesions, planned coronary bypass graft

Reasons for carotid stenting:

  • Severe coronary insufficiency, bilaterality, respiratory disease

Risk factors and medical history

  • Smoking habits
  • HTN
  • Aortic stenosis
  • Coronary angiography : 3 vessels disease

Anti platelet therapy: ASPIRIN, CLOPIDOGREL

2 D echograph :

  • Severe right and left internal carotid stenosis

Planned carotid artery stenting procedure

  • Discussion of carotid site (left or right carotid stenosis) and protocol before cardiac surgery
  • Femoral approach, carotid protection under filter, evaluation of stenosis by intra vascular OCT (LIGHTLAB ST JUDE) and rotational angiography (PHILIPS), closed cells stent, femoral closing.
