Description of lesion

  • Double RCA instent restenosis:
  • 1st lesion proximal RCA
  • 2nd lesion mid RCA
  • Length > 20mm
  • Bending
  • JCTO score 2

Description of procedure

  • Antegrade approach (retrograde access impossible)
  • Failure of Progress 80. Exchange for Progress 200T
  • Use of Finecross microcatheter and then Cross Boss
  • Exchange guidewire for Fielder XT using back the Finecross
  • Trapping technique to remove the Finecross
  • Predilatation with a 2x30mm anchoring balloon (to increase the support)
  • 3x38mm DES (distally)
  • Use of BHW guidewire and crossboss microcathter (for the 2nd lesion) Predilatation and stenting (3.5x32mm DES - proximally)
  • 24 hours later - observation and comment:
  • No stenting in the distality of the artery
